Terms of service.
Effective Date of Services: January 26
1. Effective Date
The services provided by Battling Barriers ABA Services ("Provider") will commence on the date specified in this agreement, following the signing of the contract by both parties.
2. Initial Consultation
The effective date will be contingent upon the completion of an initial consultation, during which the Provider will assess the needs of the child and develop a customized treatment plan.
3. Scheduling of Services
Upon signing this agreement and completing the initial consultation, the Provider will work with the client to establish a schedule for services. Services will begin as agreed
upon in the scheduling process, provided that all necessary information has been provided by the client.
4. Duration of Services
The duration of services will be outlined in the attached service agreement, which will be reviewed and agreed upon prior to the commencement of services.
5. Modifications to Start Date
If there are any modifications to the start date due to unforeseen circumstances, both parties will communicate promptly to reschedule and confirm a new effective date.
6. Termination of Agreement
This agreement can be terminated by either party in accordance with the termination clause outlined in this contract. In such cases, the effective date of services will cease as of the termination date.
7. Cancellation Fees
1. 24-Hour Notice: - If a client provides notice of cancellation or rescheduling at least 24 hours before the scheduled appointment, no cancellation fee will be charged.
2. Less Than 24-Hour Notice: - Cancellations or rescheduling requests made less than 24 hours before the appointment may incur a cancellation fee of [insert dollar amount, e.g., $50].
3. No-Show: - If a client fails to attend a scheduled appointment without prior notice, a no-show fee of $50 will be charged.
4. Repeated Cancellations: - If a client cancels three or more appointments within a month, Battling Barriers ABA Services reserves the right to assess a fee of $75 for each subsequent cancellation.
5. Payment of Fees: - Any cancellation fees incurred will be added to the next invoice and are due upon receipt.
